Glossary and Acronyms of E-Health related Terms

Health Information Technology (HIT)
The application of information processing involving both computer hardware and software that deals with the storage, retrieval, sharing, and use of health care information, data, and knowledge for communication and decision making.
Electronic Medical Record (EMR)
An electronic medical record (EMR) is a computerized, digital record of a patient's medical care.
Electronic Health Record (EHR)
The Electronic Health Record (EHR) is a longitudinal electronic record of patient health information generated by one or more encounters in any care delivery setting. Included in this information are patient demographics, progress notes, problems, medications, vital signs, past medical history, immunizations, laboratory data, and radiology reports. The EHR automates and streamlines the clinician's workflow. The EHR has the ability to generate a complete record of a clinical patient encounter, as well as supporting other care-related activities directly or indirectly via interface—including evidence-based decision support, quality management, and outcomes reporting.(Health Information Management Systems Society’s).
Personal Health Record (PHR)
A health record that is initiated and maintained by an individual. An ideal PHR would provide a complete and accurate summary of the health and medical history of an individual by gathering data from many sources and making this information accessible online to anyone who has the necessary electronic credentials to view the information.
Office of the National Coordinator for Health Information Technology (ONCHIT)
ONC is the principal Federal entity charged with coordination of nationwide efforts related to the implementation and use of electronic health information exchange. ONC provides counsel to the Secretary of HHS and Departmental leadership for the development and nationwide implementation of an interoperable health information technology infrastructure. Use of this infrastructure will improve the quality, safety and efficiency of health care and the ability of consumers to manage their health information and health care. ONC is organizationally located within the Office of the Secretary for the U.S. Department of Health and Human Services (HHS).
Certification Commission for Healthcare Information Technology (CCHIT)
CCHIT is a U.S. Department of Health and Human Services (HHS) funded organization that serves as the recognized US certification authority for electronic health records (EHR/EMR).
Electronic Prescribing (eRx)
E-prescriptions are computer-generated prescriptions created by a healthcare provider and sent electronically to the pharmacy. The provider enters the prescription directly into a computer or handheld device at the point-of-care. These e-prescription systems can be stand alone or part of an EHR. The privacy of personal health information contained in the prescriptions is protected by the Health Insurance Portability and Accountability Act (HIPAA).
Health Information Exchange (HIE)
Is the electronic movement of patient health-related information among organizations (known as “stakeholders”) according to nationally recognized standards. Stakeholders can include: hospitals, physicians, pharmacies, imaging centers, labs, other providers (nursing homes, rehab, etc.), payers, patients, IPA’s, or others related entities.
Health Information Organization (HIO)
An HIO is responsible for the oversight, governance, and technology responsible for the exchange of this information.
Health Care Efficiency and Affordability Law for New Yorkers (HEAL NY)
I New York State passed the Health Care Efficiency and Affordability Law for New Yorkers Capital Grant Program in 2004, often referred to as the HEAL NY Program. One of two primary objectives of the HEAL NY program includes implementing a 21st Century health information infrastructure to support the delivery of high quality care. The Department has supported two competitive grant rounds for health IT. The HEAL NY Phase 1 Health IT grant awards were announced on May 24, 2006 to 26 projects totaling $52.9 million. The HEAL NY Phase 5 grant awards were announced on March 28, 2008 to 19 projects totaling $105.7 million (HEAL NY Phases 2 and 4 were not related to health IT and HEAL NY Phase 3 was withdrawn with funds allocated to HEAL 5).
